In order to have your 2024-2025 Membership Items mailed to you, you must meet the following:
  • You live more than 40 miles from The Ugly Mug
  • You will pay $10 shipping costs
To complete the form below, you will need your MCFC Cityzen number. You can find this at: https://www.mancity.com/feed

Once your request has been received and accepted, you will receive a $10 invoice from PayPal. Once this invoice is paid, you items will be shipped as soon as possible.
